For just square root (as opposed to cube root, etc.) you don't need the "[ ]". And the 8, assuming it is supposed to be in the square root goes inside the braces:

\sqrt{8} gives [itex]\sqrt{8}[/itex]

\sqrt[3]{8} gives [itex]\sqrt[3]{8}[/itex]
