In summary, the majority of boys will stop growing between the ages of 16 and 18, after experiencing a major growth spurt during puberty that usually lasts about 2 years. However, there is a lot of variation in growth patterns, and some boys may continue to grow at a slower rate until their early twenties.

At what age do most boys stop growing, and, if one has already experienced the major growth spurt during puberty, at what rate will he continue to grow?

I looked a little on google, but most of the results seemed to be sites selling growth hormone of dubious quality.

This following was paraphrased from http://www.hgfound.org/patterns.html site. The full discussion can be read there.
The increased growth rate that goes along with adolescence begins at about age 11 in girls and 13 in boys. This pubertal growth interval usually lasts 2 years. Growth ceases between 16 and 18 years of age, when the growing ends of the bones fuse

The growth described at the foundations site may be typical or average, but there is a lot of variation. For instance I continued growing at a constant rate from about 11 to 21, about one inch every two years.
