Programs How to start a PhD?

Hi, I am currently applying for some PhD positions and am looking for a bit more information on what's involved. I know its a massive undertaking and will but alot of work but what about the specifics. That is things like how do you go about planning what work to do, how often are you expected to publish something etc. Can anyone who has done, or is currently doing one, shed some light on this.
