Does anyone know of a source for lab write-up guidelines? If so, what is the name of the textbook or URL?

Ask your professor for guidelines before reading anything else. Most professors have a particular format they want. After that, ask to look at the lab reports that other people who have taken the class before want. If you want to make it professional, look at arxiv formatting, since that's pretty close to Physical Review Letters.
