But maybe the biggest error maybe, was design:
People are known to make mistakes in panic. Design should not facilitate that. Just another variation of Murphy's law. No?
A bungling helmsman sank The Titanic when he steered right to avoid an iceberg instead of left, it was claimed yesterday.
...
quartermaster Robert Hitchins spotted the iceberg only for a steering blunder to send it smashing into it.
..
But by the time the error was spotted it was too late to avoid the iceberg. Lady Patten also revealed grandfather and second officer Charles Lightoller claimed 1,512 lives were needlessly lost as White Star Line boss Bruce Ismay ordered it to sail on to save face - which led to it taking on water.
But maybe the biggest error maybe, was design:
... In 1912 two systems, rudder for steam and tiller for sail, were used. In tiller you steered the opposite way you wished to turn.
People are known to make mistakes in panic. Design should not facilitate that. Just another variation of Murphy's law. No?
