If the embryo is expelled prior to viability, yes, it's a miscarriage (also called a spontaneous abortion). If only one of a set of twins was lost, it may or may not be possible to save the other twin. Only the woman's doctor can advise on that.

Heavy bleeding after a miscarriage or birth is normal (it is termed lochia) since all of the accumulated blood and fetal membranes during pregnancy need to be expelled, and this is much more than is built up during just a normal menstrual cycle. This is a sign that the pregnancy has ended, not a sign that there is another fetus present.

But, if she has any doubts at all that there is something abnormal about it, it is best to consult a physician, as excessive bleeding can be a sign of a life-threatening complication.

Here is a site that explains in simple terms what lochia is and what the warning signs are of complications that require contacting a physician immediately (if she at all feels light-headed/dizzy, call 911 in the US, or otherwise get her directly to the Emergency department of your nearest hospital, as this along with heavy bleeding can be a sign of dangerous amounts of blood loss).

What makes you think there's a second fetus?

I would assume that there were 2 fetal heartbeats (if the pregnancy was far enough along), but ultrasound would definitely show what's what.
