I think you should take your mother to see a doctor for assessment coz there are a number of reasons for these behaviours. The doctor will take a more thorough history, do physical exams and appropriate investigations before s/he can make a correct diagnosis and help your mother.

My friends prepared TOEFL by doing lots of sample tests or past test papers. There should be lots of resources in libraries. I think TOEFL score is needed if you want to study in the UK as well, not only in USA. TOEFL isn't a tough exam if you have good English foundation. Good luck.
