General question about Grad school.

I'm a sophomore so I'm not going to be looking into gradschool for a while, but I'm just curious how it works.

When you apply, do you apply specifically to work with a certain professor or team (maybe you are interested in their work), or do you just apply similar to how you would for an undergrad and are given a choice of teams which have positions available, or do you apply and get accepted like an undergrad, and then get assigned a team or professor to work under?

In other words, how do you end up with the research topic you end up with?
