How to quantize electromagnetic field in media?

How to quantize electromagnetic field in media? Especially in the case where the medium is dispersive and anisotropy.

To be honest, I haven't seen or done this yet, but I would suppose that you quantize the electromagnet field is the same way as in vacuum. The main difference could be that you replace the electron-positron field as known from QED by a mean-field approximation, static = infinitly heavy charges or something like that.
 
Does somebody know how to write down the lagrangian for this problem?

If we release an electron around a positively charged sphere, the initial state of electron is a linear combination of Hydrogen-like states. According to quantum mechanics, evolution of time would not change this initial state because the potential is time independent. However, classically we expect the electron to collide with the sphere. So, it seems that the quantum and classics predict different behaviours!
