Insert Images: Why Don't I See the Toolbar Button?

How come I can insert images (not attach) into my posts only sometimes? Sometimes I see the button on the toolbar and other times I do not. Also, fails to work when I can't see the toolbar button.

If your posting images not in GD with tag it won't work.
