What is a wormhole and how does it relate to the universe and space?

Need explanation on wormhole

Hello frnds, i m new to this forum and having a interest in cosmology but short of knowledge. I have seen a documentary film on universe and space and came to know about wormhole, so i have following question on it-
1. From where concept of wormhole came?
2. Why scientist believe that it is shortcut for other region of universe or space.
3. Does anyone have given practicle explantion?
4. Does black hole may act as wormhole at some poin?
