What is the Failure Mode of Torqued Fittings?

Hi Group,
Roberts Brown here, owner of Roberts Precision Machine.
Machinist for 38 years, shop owner for 30 years, expertise in all machine work with particular success in complex and demanding turning work.
Failure mode of torqued fittings brought me to this forum.
